Applications shall be filled in triplicate on PSC Form 3 (Revised 2008) obtainable from all Secretaries District Service Commission offices, Secretary Public Service Commission or can be downloaded from Public Service Commission website www.psc.go.ug. Applications should be accompanied with 3 recent passport size photographs of the applicants and photocopies of certified copies all academic documents, Registration Certificates and valid Practicing Licenses, National IDCards and other relevant documents on each form.
